How can I join the program?
You can join the program through a referral from a friend, family member, doctor, or therapist, or you can contact Montrose Adult Day Health Care Center directly to learn about our services and set up an intake interview.
What is the intake process like?
The intake process involves an interview with our staff to assess your eligibility and care needs. Following this, we will arrange a home visit by a social worker and gather your medical history either from your personal physician or our staff physician.
What conditions make someone eligible for the program?
You may be eligible if you have conditions or illnesses that affect your quality of life or independence, but do not require round-the-clock care. The staff will assess your needs and create a personalized care plan.
What kind of care and services are provided?
We provide a range of services, including nursing care, physical, speech, and occupational therapy, therapeutic activities, medication management, personal hygiene training, and social services.
Is the program covered by Medi-Cal?
Yes, the program is available for all individuals eligible for Medi-Cal. For those who are not Medi-Cal recipients, alternative payment arrangements can be made based on financial circumstances.
What types of professionals are involved in my care?
Our care team includes registered nurses, physical and occupational therapists, social workers, speech therapists, psychologists, dieticians, and other specialists, all working together to create and implement a personalized care plan.
